Cancer Care Patient Navigators (CCPN)

The CCPN staff work on behalf of the CNS staff by completing the Holistic Needs Assessments (HNA) and Care Plans with Cancer patients.

The CCPN’s provide a non-clinical supportive link for the patient and can use the HNA discussion to direct patients to various supportive options.

We can offer help and support with:
  • Signpost to local services to meet your individual needs
  • Offer support to you and those around you from diagnosis, treatment and beyond a cancer diagnosis
  • Referral to supportive and financial services
  • Referral for complementary therapies

We also provide a Holistic Needs Assessment to support you with:

  • Practical
  • Family/Relationship
  • Emotional
  • Spiritual
  • Physical
  • Information or support

We will arrange a time to talk through all your needs, this can be on the phone or in person and at a time that is suitable for you.

Your Cancer Specialist Nurse (CNS) with refer to our Cancer Care Patient Navigators (CCPN) will contact you a couple of weeks after you have been told your diagnosis.
