Diabetes
Our aim is to provide both physical and psychological support, as well as promote self-confidence in safely managing diabetes on a day-to-day basis.
Teamwork is fundamental to this and the sharing of knowledge and ideas, to help best meet the needs of patients. Acting as a learning resource for other health care professionals is also crucial to our role and in promoting patient safety. Ultimately, empowerment, through patient education as well as self-belief and motivation, is the driving force behind all that the diabetes team seeks to achieve. We have included some information which we hope supports you to be able to live to the max, achieving everything you want from it.
We are always here to help so please don’t ever be afraid to contact us for the additional guidance and support you feel you need. This is available in a whole range of options to suit your needs e.g. face to face, virtual, telephone.
