How to apply to become a member or update your details

There are a number of ways to apply for FT membership.

• The easiest way is to apply on-line
• E-mail us at: FT.membership@qehkl.nhs.uk and we’ll send you an application form in the post
• You can also download an application form to print out and post to us.

Data protection - Your information

On 25 May 2018, the new General Data Protection Regulation (GDPR) comes into effect.

The Queen Elizabeth Hospital King’s Lynn NHS FT regularly communicates with its members. Unless we hear that you do not want us to contact you, we will continue to provide updates, health news, invitations to attend events and information relating to our members. The General Data Protection Regulation (GDPR) covers the protection of members regarding the processing of their personal data.

• The QEH has an obligation to protect this data
• The QEH will hold members' details on a database that is securely protected
• The information held will be controlled by QEH staff
• Members' details will be used to contact them with information relating to their membership. This will include invitations to events, regular and ad hoc newsletters and general information that we feel may be of interest to our members
• Members can update their details by emailing FTmembership@qehkl.nhs.uk, by phoning (01553 613142) or by writing to the Foundation Trust Office
• Alternatively, members can unsubscribe by using the link at the bottom of our emails. If you ‘unsubscribe’, all your personal data will be removed from our membership database (Note - this does not include patient information held by the hospital)
