Care of the ElderlyWest Newton garden

Our aim is to provide outstanding care to all older people in Norfolk and Waveney and become a national centre of excellence in the management of frailty. Through expert medical review and multidisciplinary assessment we endeavor to maximise our patients function, minimise morbidity and prevent mortality.

We provide a full range of general and specialist services for older people with medical problems. We recognise that our patients’ situations can be very complex so their physical, social and emotional needs are also addressed. The team works in close collaboration with the community, mental health and social services to ensure that care is provided in an integrated and seamless fashion.

Our inpatient services include:

  • Acute frailty assessment
  • Integrated care for those people with complex medical and social issues
  • Specialised care for those with dementia and/or delirium
    • Bone health and hip fracture care
  • Surgical liaison services for older people under the care of our surgical colleagues
  • Speicalised care for older patients with movement disorders
  • Individualised care for those approaching the end of their lives

Our outpatient services include:

  • Rapid access frailty clinics
